Pain Medicine Summary:

The majority of our pain medicine patients come to us after experiencing chronic pain persisting over six months. Our practice is recognized by the local medical community as a center of excellence for its management of complex and challenging pain cases. We believe that the goals of treatment should be meaningful to the patient and provide attention to realistic pain relief while staying focused on improving functional activities of daily living components. These goals are best accomplished via a multispecialty clinic approach.

We provide a range of minimally invasive interventions on-site, including fluoroscopic and ultrasound-guided injections, nerve blocks, or ablations, to help reduce pain, improve function, and facilitate physical therapy. If patients continue to have an impaired function or suboptimal quality of life due to their chronic pain, we assess them for possible spinal cord stimulator implantation after a successful trial or an intrathecal pain pump implantation also following a successful day trial. Both of these invasive implantable device options require an initial psychiatric clearance but often results in long term convenient pain control.

In addition to interventional pain management, we also integrate cognitive behavioral health, multiple physical therapy modalities, topical preparations, and atypical analgesics to reduce opioid dosing in our treatment plans. Our doctors only prescribe opioids to patients who have not experienced enough relief from interventional procedures and physical therapy to maintain their daily activities and quality of life. We use a conservative approach to prescribing opioids and carefully monitor patients to ensure that the medications are beneficial. It's important to remember that patients with severe long-standing pain who require opioid medications may never be “cured,” but their symptoms can be better managed while optimizing activities of daily living remains the main focus.

Our top priority is ensuring that patients use opioids responsibly, without misuse or diversion. To achieve this, we have strict Controlled Substance Policies and Pain Contracts in place. This includes completing appropriate diagnostic testing, psychological assessments to evaluate comorbidities that may be better treated by other modalities. Risks for substance abuse potential are also addressed. Physical therapy trials to avoid the complication of deconditioning regular pill counts, and periodic review of Medication-Assisted Treatment plans. We also require regular urine toxicology screenings followed by confirmatory tests and reserve the right to demand a short notice pill count to ensure compliance.
